Tranexamic acid, also known as colic acid, or hemostatic cyclic acid, is a synthetic derivative of lysine, chemically known as trans-4-aminomethylcyclohexanecarboxylic acid. It has the chemical formula C₈H₁₅NO₂ and a molecular weight of 157.21. It is a white crystalline powder, odorless, freely soluble in water, and practically insoluble in organic solvents such as ethanol and acetone.

Application
Tranexamic acid is an anti-fibrinolytic drug that exerts its hemostatic effect by inhibiting the activity of plasmin and reducing the degradation of fibrin. It is mainly used to treat various bleeding caused by acute or chronic, localized or systemic primary hyperfibrinolysis.
Clinical Applications
Hemostasis: Used for trauma or surgical bleeding in organs rich in plasminogen activators, such as the prostate, urethra, lungs, brain, and uterus.
Anti-fibrinolysis: Used as an antagonist of tissue-type plasminogen activator (t-PA), streptokinase, and urokinase.
Others: Used for excessive menstruation with pathologically increased local fibrinolysis in the uterine cavity, anterior chamber hemorrhage, severe epistaxis, etc.

The mechanisms by which tranexamic acid improves hyperpigmentation such as melasma and freckles in dermatology mainly include the following aspects:
(1) Inhibition of Melanin Synthesis
The chemical structure of tranexamic acid is similar to that of tyrosine. It can compete with tyrosine for binding sites, interfere with the metabolic catalysis of tyrosinase on tyrosine, thereby reducing the production of melanin. In addition, tranexamic acid can also interfere with the interaction between melanocytes and keratinocytes by inhibiting the plasminogen-plasmin system, which further reduces the activity of tyrosinase.
(2) Inhibition of Angiogenesis and Reduction of Erythema
Tranexamic acid can inhibit the overexpression and activation of receptors induced by vascular endothelial growth factor (VEGF), thereby reducing the formation and dilation of skin blood vessels and alleviating the erythema symptoms in melasma.
(3) Anti-inflammatory and Repair of Skin Barrier
Tranexamic acid has anti-inflammatory effects and can reduce the expression of pro-inflammatory cytokines (such as interleukin-6 and tumor necrosis factor-α). In addition, it can also repair the damaged skin barrier by inhibiting the activation of protease-activated receptor-2 (PAR-2) by serine protease, and reduce post-inflammatory hyperpigmentation.
(4) Reduction of Melanocyte-Stimulating Factor Release
Tranexamic acid reduces the release of melanocyte-stimulating factor by inhibiting the activity of plasmin, thereby reducing the activity of melanocytes and reducing the production of melanin.
